Forms

The Department of Records & Registrations is using DocuSign for many of our forms so there is no need to submit them via fax or email.  They will be directly submitted electronically to our office.  For the few forms that are not part of the DocuSign system please fax (239-590-7983) or scan and email to orr@fgcu.edu or send directly to a Department of Records & Registration staff member most familiar with your request.
