Requesting 2FA Token
To request a 2FA token please complete and submit the form below. Following form submission, you may pick up your token from the University Help Desk, Ben Hill Griffin Hall, Room 252. Tokens are available for a limited time on a first-come, first-served basis. Tokens are available from the Help Desk as a one-time allocation. Lost or additional tokens must be purchased at your expense; alternatively, you may opt to utilize the recommended Duo Mobile app.
Please remember to bring your photo ID when picking up a token.
